New Food Establishment Information

The Health Services Department requires a completed application and fees, and plans be submitted a minimum of 30 days in advance of anticipated opening in order to receive the required Food Service Establishment or Food Establishment License.

The following documents must be completed:
1) Submit Plans for food establishment
2) Food Establishment Application for New Establishment

An Environmental Sanitarian will review the plan review packet and inspect the facility prior to issuance of a food license. The completed documents can be turned in to the Health Services Department located at City Hall Annex, 1300 Main St, Dubuque, IA 52001.
