New Food Establishment
The Health Services Department requires a completed application, applicable fees, and plans to be submitted at least 30 days before the anticipated opening date to obtain a Food Service Establishment or Food Service License.
New food establishments must complete the food establishment application available on the City website.
An Environmental Sanitarian will review the submitted plans and inspect the facility before a food license can be issued.
Submit completed documents to the Health Services Department at City Hall Annex, 1300 Main St., Dubuque, IA 52001.
